fix(jj-describe): stop rewriting revisions that already have descriptions
An agent mistook another writer's described revision for its own work layer and rewrote it with jj desc -r, rebasing its descendants.

- check the target revision with one read-only log before mutating it
- stop and report when it is non-empty and already described
- record the agent's own work on a fresh jj new layer instead
